Undertaking advance care planning is a time for your loved one to reflect on their values and wishes, and to let you know their future health and personal care preferences so that you or their nominated representative can speak for them, if they can’t speak for themselves. It’s about the conversation, with friends and family and with your loved ones doctor.

Once complete it’s a good idea to then store the advance care plan with any document that appoints a person to make medical decisions for your loved one under the laws of your state or territory . Copies of your advance care plan should be given to the nominated representatives, members of your loved ones family and your doctor.
